Total Protocol XV: Results in children under one year old with Acute Lymphoblastic Leukemia

Abstract

Background: the clinical picture, laboratory tests and survival in patients under one year of age with a diagnosis of Acute Lymphoblastic Leukemia (ALL) differ from those of other age groups. Objective: to describe the clinical variables and overall survival (OS) in children under one year of age with ALL, treated with the Total XV chemotherapy protocol. Methodology: cohort study, which included infants under one year old diagnosed with ALL and treated with the Total XV protocol, at the SOLCA-Cuenca Cancer Institute (Ecuador), in the period 2006-2017. The dependent variable was OS and the independent variables were: biodemographic, clinical, translocations, immunophenotype, initial blood count and minimal residual disease (MRD). Results: 8 patients (6 girls) were studied, with a median age of 11 months. All had positive DHL (>300 U/L) and hepatosplenomegaly. One patient presented tumor lysis syndrome and three infiltration of the Central Nervous System; two were translocations and 7 were immunotype B. The median leukocytes, hemoglobin and platelets at the time of diagnosis were 79050 cells/uL, 6.8 g/dl and 46600 cells/uL respectively. Negative MRD was achieved on day 15 and day 42 of the induction phase in six patients. OS after a median follow-up of 89 months was 50%; There were 3 deaths in complete remission (CR) and one without CR. Conclusion: the clinical characteristics and OS observed are similar to similar reports, but inferior to those described in other age groups.
