Craniofacial Fibrous Dysplasia Involving the Paranasal Sinuses: A Case Report

Fibrous dysplasia is a benign, congenital bone disorder in which normal medullary bone is progressively replaced by immature fibro-osseous tissue. Although biologically benign, craniofacial involvement can clinically and radiologically mimic destructive neoplasms due to its capacity to expand into the paranasal sinuses. We report the case of a 22-year-old female presenting with chronic left nasal obstruction and diffuse facial pain. Computed tomography of the paranasal sinuses revealed a homogeneous, expansive endoosseous lesion with characteristic ground-glass density (411 HU) involving the frontal sinus, ethmoid, sphenoid, crista galli, and left orbital wall, which caused a reactive severe "italic S" septal deviation. An endoscopic decompressive biopsy was performed. Histopathological examination confirmed a fibro-osseous proliferation composed of irregular, curvilinear immature bone trabeculae arranged in a "Chinese characters" pattern within a uniform fibrous stroma, characterized by the diagnostic absence of osteoblastic rimming and cellular atypia. The close correlation between the non-destructive expansive tomographic pattern and classic histological findings established the diagnosis, ruling out osteosarcoma and supporting conservative management. Anatomoradiological recognition of this multisinusal entity prevents unnecessary radical surgical interventions.
