Resumo

Introdução: As fraturas do tálus e do calcâneo, embora menos frequentes do que outras lesões do pé, representam um desafio cirúrgico devido à sua anatomia complexa e à frequente associação com mecanismos de alta energia. O tratamento tradicional por meio de redução aberta e fixação interna (RAFI) tem sido associado a uma elevada taxa de complicações. Nesse contexto, a artroscopia surgiu como uma alternativa terapêutica que permite a visualização direta da superfície articular e potencialmente reduz a agressão aos tecidos moles. Objetivo: Avaliar os resultados clínicos, as considerações técnicas e as indicações do tratamento artroscópico das fraturas do tálus e do calcâneo. Métodos: Foi realizada uma revisão narrativa da literatura nas bases MEDLINE/PubMed, EMBASE e Cochrane Library, incluindo estudos publicados entre 2000 e 2024 sobre o tratamento artroscópico das fraturas do retropé. Resultados: As técnicas assistidas por artroscopia apresentam vantagens potenciais, como melhor visualização da superfície articular, menor agressão aos tecidos moles e possibilidade de recuperação mais rápida em comparação com a RAFI, especialmente em fraturas selecionadas. Conclusão: A artroscopia constitui uma alternativa viável para fraturas selecionadas do tálus e do calcâneo, particularmente em fraturas fechadas com comprometimento articular e condições favoráveis dos tecidos moles.
