Hemodynamic Monitoring in Critically Ill and Perioperative Patients: An Update and Emerging Tools

A monitorização hemodinâmica evoluiu de uma abordagem centrada em variáveis macrocirculatórias invasivas para uma estratégia mais abrangente, voltada à avaliação da perfusão tecidual, da função vascular por métodos não invasivos e à predição precoce da deterioração circulatória. Esta revisão narrativa sintetiza as evidências recentes sobre ferramentas aplicáveis a pacientes críticos e no período perioperatório, com ênfase na microcirculação, na monitorização contínua da pressão arterial sem acesso arterial invasivo e nos modelos preditivos baseados em inteligência artificial. A literatura demonstra que a disfunção microvascular pode persistir apesar da aparente correção das metas sistêmicas e fornecer informações prognósticas adicionais. Paralelamente, as tecnologias cuffless e multimodais ampliaram as possibilidades de monitorização contínua, embora persistam limitações relacionadas à calibração, precisão e reprodutibilidade clínica. Os modelos preditivos têm aprimorado a detecção precoce da hipotensão intraoperatória; entretanto, seu valor clínico depende de sua integração à tomada de decisão fisiologicamente orientada. Em conjunto, essas ferramentas estão redefinindo a monitorização hemodinâmica como uma estratégia multimodal, dinâmica e individualizada.Downloads
