Hemodynamic Monitoring in Critically Ill and Perioperative Patients: An Update and Emerging Tools

Abstract Hemodynamic monitoring has evolved from an approach centered on invasive macrocirculatory variables to a broader framework focused on tissue perfusion, noninvasive vascular assessment, and early prediction of circulatory deterioration. This narrative review summarizes recent evidence on tools applicable to critically ill and perioperative patients, with emphasis on microcirculatory monitoring, continuous noninvasive blood pressure assessment, and artificial intelligence based predictive models. Current literature shows that microvascular dysfunction may persist despite apparently corrected systemic targets and may provide additional prognostic information. At the same time, cuffless and multimodal technologies have expanded the possibilities for continuous monitoring, although important limitations remain regarding calibration, accuracy, and clinical reproducibility. Predictive models have improved the anticipation of intraoperative hypotension, but their usefulness still depends on integration with physiologically guided clinical decisions. Taken together, these advances support a transition from isolated parameter tracking to a multimodal, dynamic, and individualized hemodynamic monitoring strategy.
